Pixel Mania Super Fly Box Maker
As the original Pixel Mania site is down I have found my copy and uploaded it here:

I dont have a copy of the original tutorial but you basically need to;
- create a new photoshop file 500 x 500 pixels
- create 2 layers 1 named "front"
- The other layer "Side"
- Create your box flat on the screen (use the template as a guide in the zip file above)
- Import the action and press play, instant box.
